NSW: Communication breakdown and faulty track behind Hexham=2
NSW opposition transport spokesman Peter Debnam later said the findings repeat thefaults raised in the Glenbrook disaster inquiry.
Seven people died when their interurban passenger train crashed into the back of theIndian Pacific 400 metres from Glenbrook Station, in the Blue Mountains, in December 1999.
Mr Debnam said the root cause of the Hexham accident went back for several years andwas related to the break up of the state rail system in 1996.
"Training issues, communication issues, lack of compatible radios, lack of communicationbetween train drivers and concerns between signallers and network control ... there isa long string of difficulties that led to the crash on July 12," Mr Debnam said.
